When an individual seeks copyright permission for a particular work the request should be directed to the publisher's copyrights and permissions department, not the author. The following information should be included in the request for copyright permission:
* Author's, editor's, translator's full name(s) * Title, edition and volume number of the book or journal * Copyright date * ISBN for books, ISSN for periodicals * Numbers of the exact pages, figures, and illustrations to be copied. * If you are requesting a chapter or more; both exact chapter(s) and exact page numbers. * State whether the material will be used alone or combined with other photocopied materials. * Name of college or university you are affiliated with * Course name and number in which the material will be used. * Semester and year in which material will be used. * Instructor's full name.

HOW DO I FIND OUT WHO OWNS THE COPYRIGHT FOR A PARTICULAR WORK?
In order to determine who legally owns the copyright for a particular work an individual should consult both the page containing the copyright notice as well as any acknowledgment pages in the work. Most works contain a notice of copyright although such notice is not required for copyright protection. The information contained in such copyright notice may not always reflect accurately the identity of the current copyright owner of the material in question. The best method for determining copyright ownership is to contact the publisher of the work that you wish to copy.
